By signing this form, I, the parent/guardian acknowledge and agree to the following terms and conditions for participation in the Secondary School Basketball League:

I understand that all players must complete and sign this consent form to participate in the High School League. Failure to do so will result in the player being unable to participate in any games or league activities.

I understand that games may be cancelled due to severe weather or unsafe conditions. If a game is cancelled, there is no guarantee that it will be rescheduled or made up at a later date.

I understand that players are required to wear team uniforms during all games. Teams may either wear MusFit’s official uniforms (included with registration) or their school’s designated team uniforms, if provided by the school.

I understand that respectful behavior is expected at all times. Any player found to be using disrespectful language, profanity, or inappropriate behavior towards MusFit staff, referees, facility members, opposing coaches, or players may be disqualified from the league. Reinstatement will be at the discretion of the League Manager.

I understand that parents/guardians are responsible for the pick-up and drop-off of their child to and from the designated game and practice locations. The school and league organizers are not responsible for student transportation before or after the scheduled activities.
